MEMORIAL DONATION

A donation to WLMA is a positive way to commemorate the life of a loved one.

If you would like people to make a donation to WLMA in lieu of flowers, we suggest you include a request in your loved one’s death notice.

TO MAKE A MEMORIAL DONATION:

STEP 1

We accept 3 payment methods: E-transfer, Cheque or Credit Card.

Please fill out the Memorial Donation Form if you are paying by E-transfer or cheque (link below).

Pay and submit the Memorial Donation form based on your selected payment method.

Complete the e-transfer to WLMAFinance@bell.net and either mail OR drop off the Donation Form to our Auxiliary Office located in the West Lincoln Memorial Hospital OR email the form to WLMAFinance@bell.net

* Please remember to provide the e-transfer password when returning the Donation form.

Cheque:

Mail or drop off your cheque and the Donation Form to our Auxiliary Office located in the West Lincoln Memorial Hospital.

Credit Card:

Complete your payment using the link below (Canada Helps). Canada Helps collects donor information as part of the payment process.

CANADA HELPS

Once we receive your donation, we will send you a letter of thanks along with a tax receipt. We will also send a notification to the family of the honouree. We do not disclose the amount of the gift in this notification.
